This form will be filled out and signed by parishioners that may be photographed or filmed for a parishes facebook, websites or general PR purposes. You may not photograph or film someone without their permission. These forms will be submitted to the parish they attend.
Youth Acknowledgement Policy Form
This form is for anyone leading a youth program, such as sunday school or a summer camp. It will be signed and returned to the parish the program is taking place at.
This form is for participants in youth programming. These forms will be filled out prior to the beginning of each program and kept in a secure place at that parish, when not with the teacher who is leading the program. For the duration of the program, these participant forms will always be in the room where the program is taking place and readily available if needed in the event of an emergency.

The A-1 and A-2 Policy Acknowledgement Forms will be signed as required by volunteers and employees. Please ask your safechurch liaison for guidance or email safechurch@bc.anglican.ca.
This form will be use for Youth Programs to gather important information in case of an emergency.
Template for Letters to the Police for PICs (volunteers)
This is a template for each parish to use when a volunteers needs to go to the police station to obtain a volunteer criminal record check so the police waive the fees. Please note: All PIC fees are the responsibility of the parish and the police will not waive the fees unless a letter is provided with proof of their volunteer status.
